Changes to SMF element activity data with API and CSV utility after RO58777 or RO55818

Issue/Introduction

There were changes made to both the API and CSV utility formatted data for SMF element activity. The target (TO) fields were overwriting the source (FROM) fields and the source (FROM) fields were missing from the CSV SMF Function. Additionally, many of the actions were not correctly writing the API SMF data. For example, the DSN/MBR field were not being written for the ADD/RETRIEVE actions. P6083

Resolution
