When creating an incident, the customer gets an error screen, rather than the screen to enter the incident.

Incorrect URL reference in the Service Catalog
The reference needs to be changed in the service catalog.
1. Click on Admin>Service Catalog Settings
2. Go to the correct category where the failing reference is
3. Find the entry and click the gear icon on the right side
4. Click Edit Form
5. Change the URL reference at the top of the page. It should be something like:
/SD.IncidentManagementSimple.SubtaskTemplates
NOT
Https://servername.domain/SD.IncidentManagementSimple.SubtaskTemplates
6. Click Save
