When raising a request to the catalog offering , pay attention to system's "General Information" section . It has a "Date Required" field there . If you select a date into "Date Required" field first , and then click Attachment(0) or Note(0) , it is not responding .
If you try above again with "Date Required field" empty , then it will work .
