Upon completion of the questionnaire, please reach out to your Account Director or Customer Success Manager to have a Hot Site created with the support organization for awareness.
Completing this form with accurate responses is highly recommended to receive the highest level of support if you encounter any P1 issues during your upgrade. Please have the form completed at least 2 business days prior to your upgrade.
