Threshold profile rule with multiple conditions is not triggering

Cause

If you have multiple conditions, all must be met for the rule to trigger. 
 

Resolution

If you need to match multiple conditions independently, use multiple rules in the profile, one for each independent condition you need to match. 

Having multiple conditions is valid, but only if you need all to match for the particular rule to trigger.
