Data Forms not creating event

Issue/Introduction
After creating a Data Loading Form, events are stuck in "Pending" status. Using the "Load Pending Records" option is not effective - the records remain stuck.
Environment
CA Business Service Insight 8.3.x and 9.x
Cause
In the t_log table, there is an error of the following form:
error : "Name" attribute in "AdapterConfiguration/InputFormatCollection/InputFormat/InputFormatFields/InputFormatField[4]" node is empty or contains illegal characters ("SLA Applicability"). Legal characters are A-Z, a-z, 0-9 and _ .
Resolution
As per the error message, the input field names for Data Mapping cannot contain spaces and can only contains alphanumeric characters. Correcting this will allow the form to work.
