Reports / Query Service keeps crashing

Issue/Introduction:

While running reports, the query service crashes and a "Service Restarted" or "Failed to complete Baseline Calculation: The database request has failed" error displays in the Custom Reports page.

Environment:

NFA 9.3.6
NFA 9.3.8
NFA 9.5.0

Resolution:

Follow the instructions inside the patch to apply.
